Evvy Vaginal Microbiome Test

Published June 2026  ·  v1.0 — original

Evvy is the only DTC vaginal health test using true metagenomic sequencing — meaning it doesn't look for a preset list of bacteria, it reads everything present. That difference matters. Traditional testing misses a significant portion of the microbial landscape. Evvy doesn't. And unlike most diagnostic tests, results come with a clinician review, a care plan, and a path to Rx treatment — all from one swab.

Sequencing methodmNGS (metagenomics) — reads all DNA present
Microbes tested700+ bacteria and fungi
vs. PCR / 16S testsThose test a predefined list only — Evvy tests everything
Covers BV pathogensYes — Gardnerella, Atopobium, Prevotella, and more
Covers yeastYes — C. albicans, C. glabrata, other Candida species
Covers protective bacteriaYes — L. crispatus, L. gasseri, L. iners
Fertility insightsYes — diversity score, fertility disruptors
Covers STIsOnly with Expanded PCR Panel add-on (+$89)
Functional / metabolic dataNot measured — organisms only
The mNGS approach is a genuine technical leap over the 16S rRNA or PCR panels used by every other DTC vaginal health test. It catches organisms that predefined panels miss — particularly less common BV-associated species and rare fungal infections. The limitation is that it tells you what's there, not what those organisms are doing. No metabolic or immune markers. Still, for a single swab that answers "what's in my vaginal microbiome," this is the most complete answer available anywhere, DTC or otherwise.

Lab certificationCLIA · CAP · CLEP (triple certified)
Peer-reviewedYes — validation study published
Validation resultImproved analytical sensitivity and specificity vs. traditional methods
Medical oversightMedical Advisory Board — Stanford, UCSF, Northwestern OB-GYNs
Physician orderingYes — physician-ordered and reviewed per CLIA requirements
IRB-approved researchYes — opt-in data contributes to gender health gap research
Triple certification and a peer-reviewed validation study are real. The medical advisory board is credible — these are not figurehead appointments. The limitation keeping this below 8.0: published clinical outcomes data specifically on the Evvy product (e.g., sensitivity/specificity vs. Nugent criteria, or BV recurrence rates with Evvy-guided treatment) is limited in the public literature. The mNGS methodology is validated; the question of how much better outcomes are with Evvy-guided care vs. standard care hasn't yet been demonstrated in published RCTs.

Custom care planYes — included with every test
Rx accessYes — for BV, yeast, AV, and more (if eligible)

Trend trackingYes — with membership (quarterly testing)
Results complexityHigh — 700+ microbes requires interpretation guidance
Rx availabilityUS (some state restrictions); not available in Canada
The physician review + care plan + Rx access pipeline is the most integrated DTC vaginal health experience available — and it fills a gap that traditional OB-GYN visits miss (the thoroughness of the microbiome data). The coaching call is a meaningful differentiator for navigating complex results. Deductions for: Rx not included in price (real added cost), state-by-state availability limitations, and the inherent complexity of 700+ microbe results that can be overwhelming without the coaching walkthrough.

OB-GYN comparisonVisit + standard lab + copay typically $150–300+
At $129–159 per test, the value case depends heavily on how you're using it. For someone with recurrent BV or yeast infections cycling through antibiotics, the comprehensive data + Rx pipeline is likely worth more than the cost — especially vs. repeated $35+ copay clinic visits that often don't resolve the root cause. HSA/FSA eligibility helps. But for a proactive, symptom-free person testing annually, $159 is a meaningful expense. The quarterly membership model assumes ongoing engagement that not every user will sustain. Rx costs adding on top further increases the total spend.

The at-home swab is straightforward and the results portal is well-built — you can filter your microbiome results by condition (BV, yeast, fertility, etc.), which makes the 700+ microbe output navigable rather than overwhelming. The coaching call is a genuine differentiator. The main friction point: 7–10 business days for full results is slow. If you're symptomatic and waiting for answers, that's a long time. The Expanded PCR Panel's 1–3 day prelim helps, but that's an $89 add-on. For a proactive user with no urgent symptoms, the wait is fine.
Why standard vaginal testing consistently fails Traditional vaginal swabs at the OB-GYN use PCR or culture methods that check for a predefined list of maybe 10–20 organisms. Evvy's mNGS reads the entire genome of everything in the sample — so a recurrent "BV" patient who's actually dealing with aerobic vaginitis caused by a less common pathogen gets an answer instead of another round of metronidazole that won't work. The same logic applies to unexplained symptoms: if you've been tested for the usual suspects and come back negative, a comprehensive metagenomic readout often finds what's been missed.

MYM verdict
"If you've had recurrent BV, yeast infections, or unexplained vaginal symptoms that haven't responded to standard treatment, Evvy is the most rigorous first step available without a specialist referral. The metagenomics difference isn't marketing — it catches organisms that every other test misses. The clinician review and Rx pathway mean you get answers and a plan, not just a spreadsheet of bacteria. The tradeoffs are real: it's expensive for ongoing use, results take a while, and Rx is an added cost. But for someone finally trying to understand what's actually going on, there's nothing else like it."
