Marek Diagnostics Comprehensive Panel

Published June 2026  ·  v1.0 — original

Marek Diagnostics is the testing arm of Marek Health, built for people who care about methodology, not just results. The Comprehensive Panel measures ~80 biomarkers — but what separates it is how it measures them. Total testosterone uses LC/MS (liquid chromatography-mass spectrometry), the reference method used in research settings. Free testosterone uses equilibrium ultrafiltration, not the calculated or analog assay most labs rely on. Testosterone ranges are uncapped, so if you're optimizing into the 1,500+ ng/dL range, the number isn't cut off. If you've ever been told your labs are "normal" and suspected otherwise, the issue may be the method — and Marek fixes that.

~80 biomarkers is strong at the Comprehensive tier, and the panel covers nearly every category a health optimizer would care about. ApoB and Lp(a) are included — most competitors leave those out of base panels. The sex hormone methodology alone (LC/MS + equilibrium ultrafiltration) justifies a premium rating here. Fewer total markers than Function Health (160+) at this tier, but the methodology on the markers that matter most is superior.

Why this mattersStandard immunoassays overestimate testosterone at low values, underestimate at high values, and cross-react with other steroids
This is Marek's defining strength. Standard testosterone immunoassays — used by most DTC labs and the majority of conventional physicians — are notoriously imprecise, especially at low ranges (relevant for women) and high ranges (relevant for TRT). LC/MS eliminates that imprecision. Equilibrium ultrafiltration for free testosterone is the reference method endorsed by the Endocrine Society; calculated free T and analog assay are widely criticized in clinical literature. Marek uses the right methods. That's a rare thing to say about a DTC lab.

Actionability at the base Comprehensive Panel tier is limited — you get accurate results and a LabCorp portal, but interpretation is on you unless you book the optional 45-minute coach review. The full Marek Health platform (separate subscription) provides ongoing coaching and treatment optimization, which is excellent — but that's a different product and a different cost. Compared to Function Health or Superpower, which include built-in coaching and action plans in their base price, Marek's standalone testing tier scores lower here by design.

Vs. SuperpowerSuperpower: $349/yr for 100+ markers (but immunoassay hormones)
À la carte optionLab builder lets you add individual markers — pay only for what you need
At $495 per test, Marek is expensive by DTC standards. Function Health's annual membership ($365/yr) covers 160+ biomarkers. Superpower covers 100+ for $349/yr. But neither uses LC/MS for hormones. The premium is real and the reason for it is defensible — you're paying for methodology, not marketing. For someone focused specifically on hormone optimization or TRT/HRT monitoring, the extra cost is worth it. For general annual health screening, cheaper alternatives offer more markers per dollar.

The ordering experience is smooth and the LabCorp footprint is excellent — 2,000+ locations means most users can find a draw site within a reasonable distance. The results arriving in a LabCorp portal (rather than a dedicated Marek dashboard at the base tier) is the main friction point. No app, no trend tracking, no visual dashboard unless you're also enrolled in the full Marek Health platform. For a product priced at $495, a more polished standalone results experience would strengthen this score.
Why methodology matters more than most people realize Standard immunoassay testosterone tests — used by most DTC labs and most primary care physicians — are accurate enough for population-level screening. They are not accurate enough for the optimization decisions most Marek customers are trying to make. LC/MS eliminates cross-reactivity with other steroids, is more precise at low and high ends of the range, and produces the uncapped readings that matter for TRT monitoring. If your doctor said your testosterone is "fine" at 350 ng/dL on a standard assay, that number may not be what you think it is.
Pros & cons
What works
+ LC/MS for total testosterone and estradiol — the gold standard, not the default
+ Free testosterone via equilibrium ultrafiltration — the Endocrine Society's reference method
+ Uncapped testosterone ranges — accurately reports values above 1,500 ng/dL
+ ApoB and Lp(a) included in the base Comprehensive panel
+ Full iron panel (ferritin, TIBC, transferrin saturation, UIBC)
+ À la carte lab builder — add individual biomarkers without buying a bigger panel
+ HSA/FSA eligible via TrueMed
+ 2,000+ LabCorp draw sites — broad access nationwide
Watch out for
− $495 is expensive for a one-time test — Function and Superpower offer more markers per dollar (with immunoassay)
− No built-in coaching or action plan at base tier — results land in a LabCorp portal
− No dedicated app or trend dashboard without enrolling in Marek Health platform
− Only one vitamin tested (Vitamin D) — no B12, magnesium, omega-3, zinc
− LabCorp only — no Quest option
− Strong identity as a hormone/TRT platform — less built out for women's health focus
Bottom line
MYM verdict
Best-in-class hormone methodology for DTC testing. LC/MS and equilibrium ultrafiltration aren't marketing claims — they're the methods clinicians use when precision actually matters. At $495 for the Comprehensive panel, Marek costs more than alternatives, but for anyone monitoring TRT, HRT, or hormone optimization, the extra precision is worth the premium. If you want 160 markers and an action plan in a slick app, Function Health is the better call. If you want to know what your testosterone actually is, Marek is.
